Date Assigned
Enter the date the investigator was assigned to the complaint case.
Date Investigator Report Completed
Enter the date the investigator finished and turned in her/his report on the results of the investigation of the complaint.
Examiner
Assigned to

Date Assigned
Enter the date the examiner was assigned to the complaint case.
Date Examiner Report Completed
