Individual Conviction Tab
This tab contains all of the basic information on Convictions filed against the individual.
This screen only applies for Mortgage Brokers and Loan Officers.
Field Descriptions
CRIMINAL HISTORY INFORMATION
Date
This is the date the crime was reported.
Offense
Enter the official criminal offense the individual was charged/convicted. This field includes up to 50 characters.
Crime Type
Enter a user defined code up to 20 characters for the type of criminal offense. Examples: Charge, Conviction, etc.
Authority
List the governmental entity the charge/conviction is filed with.
Case No
This is the number assigned by the governmental agency. This field includes up to 30 characters.
Renewal Affidavit
Enter Y for yes or N for no.
Outcome
Briefly describe the outcome of the charge/conviction. In addition, this note field is a true rich text format - which includes the ability to spell check, add formatting, bullets, etc. You can double click on this field to edit by cutting and pasting text from another application.

Enter Y for yes or N for no.
Charges Pending?
Enter Y for yes or N for no.
Civil Lawsuits?
