Individual Personal Tab (FSA)
This tab contains all of the personal information for each Financial Services Agent.
This screen only applies for Financial Services Agent. There are different screens for other Record Types.
Field Descriptions
Age
The MyLIC program will display the correct age according to the Date of Birth entered.
DOB
Enter the Date of Birth for this individual.
Personal History
Conviction?
Enter Y for yes, if the individual has one or more criminal if not enter N for no convictions.
Sentence Imposed?
Enter Y for yes if the individual has had a sentence imposed on him/her if not enter N for no.
Charges Pending?
Enter Y for yes if the individual has any charges pending if not enter N for no.
Civil Lawsuits?
Enter Y for yes if the individual has had any civil lawsuits filed against her/him if not enter N for no.
Existing License?
Enter Y for yes if the individual has an existing license with another state if not enter N for no.
Regulatory Ban?
Enter Y for yes if the individual has a regulatory ban filed against her/him if not enter N for no.
Background Check
Finger Print Card?
Enter Y for yes if the individual has a finger print card on file if not enter N for no.
Fast Pass?
Enter Y for yes if sending for fast pass (Located only in Texas) and N for no if finger print was provided.
Sent
Enter the date the background check request was remitted.
Rcvd DPS
Enter the date the DPS background report was received.
Rcvd FBI
Enter the date the FBI background report was received.
User
The MyLIC program automatically enters the initials from the User table whenever the record is added or changed.
Last Chg Date
The MyLIC program automatically enters the current date whenever the record is added or changed.
