Providers Branches Tab

This tab displays all of the basic information on each branch of a provider.  

Field Descriptions

Provider No

Semarca License program displays the assigned unique Provider number and transaction number. This number can not be changed and is incremented by one each time a record is saved.

Address

Enter the provider's branch address on two lines that can contain up to 34 characters each.

City

Enter the provider's branch city containing up to 20characters.

State

Enter the provider's branch state containing up to 2 characters.

Zip Code

Enter the provider's branch zip code.

Tel 1

Enter the provider's branch area code and work telephone number.
